Two Cases of Hepatosplenic Candidiasis in Children with Acute Leukemia

Abstract
Although intensive chemotherapy for malignant diseases can prolong the survival, it also induces profound immunosuppression and increase the risk of serious fungai infections. Candida albicans is the most common cause of systemic fungal
infections
and
hepatosplenic candidiasis has been increasingly recognized as a variant of dissminated candidiasis among immunocompromised host. We have experienced two cases of hepatosplenic candidiasis in children with acute leukemia. They presented recurrent
fever
and hepatosplenomegaly after recovery from neutropenia. Characteristic 'target'lesions in the liver, spleen, and kidneys were noted on abdominal ultrasonography. Each case was confirmed by culture of blood or liver aspirates, respectively. Both
were
treated successfully with amphotericin-B in combination with fluconazole. Hepatosplenic candidiasis is considered as one of the importan causes of fever unresponsive to empirical antimicrobial therapy in patients with prolonged neutropenia.
Therefore,
empirical antifungal therapy should be given to suspicious cases.
